⚠ EMERGENCY — Toxic Megacolon Colon dilation greater than 6 cm on imaging Systemic toxicity: high fever (above 38.6°C), rapid heart rate, raised white cell count Distended, tender abdomen — may indicate perforation Severe bloody diarrhoea + high fever + distended abdomen = go to emergency immediately Acute severe colitis is defined by the Truelove & Witts criteria: 6 or more bloody stools per day plus at least one systemic sign (fever, anaemia, raised inflammatory markers, tachycardia)
